Tantangan Manajemen Perioperatif Syok Hemoragik pada Kehamilan Ektopik Terganggu: Sebuah Laporan Kasus

Abstract

Kehamilan ektopik terjadi ketika sel telur yang dibuahi menempel di luar rongga rahim dan merupakan salah satu penyebab utama mortalitas pada trimester pertama. Seorang wanita berusia 32 tahun dengan G3P0030 dengan usia kehamilan 7-8 minggu yang mengalami kehamilan ektopik terganggu dengan syok hemoragik sebagai penyulit. Pasien menunjukkan gejala hipotensi berat, takikardia, dan asidosis metabolik yang diakibatkan oleh kehilangan banyak darah. Pasien dilakukan resusitasi dengan kristaloid, produk darah, dan dukungan vasopresor, operasi laparotomi dilakukan secara segera. Manajemen intraoperatif melibatkan pemilihan agen anestesi yang cermat, termasuk ketamin dan midazolam, untuk memastikan stabilitas hemodinamik. Perawatan pasca operasi di ICU meliputi penyesuaian vasopresor dan terapi cairan lanjutan, yang pada akhirnya mengarah pada stabilisasi dan keberhasilan ekstubasi. Kasus ini menunjukkan pentingnya diagnosis yang cepat, strategi resusitasi yang tepat, dan pemilihan anestesi yang cermat dalam penanganan syok hemoragik sekunder akibat kehamilan ektopik sehingga meningkatkan keberhasilan pemulihan pasien.
